Studies on Liver Scintigram in Acute and Fulminant Hepatitis


Abstract
Liver scintigram using radiogold "Au was performed in 84 p.,tients who were given definite diagnosis of acute virus hepatitis and fulminant hepatitis. The studied cases involved those who were hospitalized in the Department of Internal Medicine, Busan National University Hospital from the period of October 1962 to April 1977.
The following results were obtained:
1. The author noticed hypertrophy pattern on scan in 52 (81. 2%) acute virus hepatitis cases out of 64 cases, and atrophy pattern in 16 (80%) fulminant hepatitis cases out of 20 cases. Spleen visualization was also noticed in 33(51.6%) acute virus hepatitis cases out of 64 cases and in 14(70.0%) fulminant hepatitis cases out of 20 cases.
2. As to the correlation between scintigraphic findings and clinical pictures, hypertrophy pattern was noted in 81.2% of acute virus hepatitis and in 30.0% of fulminant hepatitis as compared to an incidence of 70. 3% clinically palpable liver in acute virus hepatitis and of 20.0% in fulminant hepatitis. Spleen visualization on scan was noted in 51.6% of acute virus hepatitis and in 60.0% of fulminant hepatitis as compared to low incidence of 10.9% of clinically palpable spleen in virus hepatitis and of 10.0% in fuhninant hepatitis.
3. Comparative serial scanning studies were carried out on six acute and fulminant hepatitis. In cases of acute virus hepatitis hypertrophy pattern showed decreases in size the 5th and the 7th hospital day and in cases of fulminant hepatitis atrophy pattern showed gradual improvement the 7th Hospital day. Spleen visualization showed decrease in grade the 9th and 10th day in both acute virus and fulminanant hepatitis.
4. It has been shown that liver scintigram not only play an important role in diagnosis of acute virus and fulminant hepatitis but it also helps in an evaluation of its healing and prognosis.
